ELIGIBILITY

All full-time employees with a normal schedule of 30-40 hours per week are eligible for the benefits described in this guide. Medical and dental benefits begin on the first of the month following 30 days of employment, while life and disability benefits begin on the first of the month following 90 days of employment. Part-time and internship employees are not eligible to participate.

If you do not enroll within 90 days of your initial eligibility period, you will not have an opportunity to do so until the next open enrollment period, unless you have a Qualifying Life Event.​

Eligible dependents include: (1) Your legal spouse; and (2) Your children up to age 26. Only those dependents who meet the eligibility requirements can enroll for coverage.

Medical

MEDICAL

With our medical plans you can access care in-network or out-of-network. We recommend accessing in-network care whenever possible, as it will be less expensive. The medical plans are administered by Independence Administrators.

HSA

HSA - HEALTH SAVINGS ACCOUNT

A Health Savings Account (HSA) combines high deductible health insurance with a tax-favored savings account. Money in the savings account can help pay the deductible. And money left in the savings account earns interest and is yours to keep.

For 2021 you can contribute up to $3,600 if you enroll as a single in the medical plan, or up to $7,200 if you enroll as a family. Additionally, if you are age 55 or older, you are eligible to make a catch-up contribution of $1,000.

For 2021, ABC will continue to make a contribution of $1,000 if you are enrolled in the Qualified High Deductible medical plan as an individual and $2,000 if you are enrolled with dependents. This amount is prorated for new employees based on your hire date within the plan year. Here is a list of common HSA eligible expenses.

This benefit is available if you want a little extra protection for those pearly whites.
The Dental plan is administered by United Concordia and offers both in-network and out-of-network coverage.
